Flatpak: Add data migration to readme

Daniel Brötzmann requested to merge wurstsalat/gajim:flatpak-instructions into master

This adds the data migration section from https://dev.gajim.org/gajim/gajim/-/wikis/help/flathub to the Flatpak readme. This effectively merges both pages, so we can link to the readme from the wiki (and make that wiki page obsolete).
