Shutdown if stream end times out

mesonium requested to merge mesonium/python-nbxmpp:dead-tcp-shutdown into master

A dead TCP connection (VPN switch, changed network, a resumed session whose socket is gone) never answers. Ensure we close our side of the stream after a grace period in this case.
